PACIFIC COUNTY — The Washington State Patrol is investigating a fatal two-vehicle collision that occurred Sunday morning, October 5, 2025, just north of Butte Creek on U.S. Highway 101.
According to a press memo from WSP, a gray 2018 Jeep driven by Rex M. Mullikin, 26, of Rock Springs, Wyoming, was traveling southbound on US 101 when it crossed the centerline and collided head-on with a blue 2007 Dodge Caravan traveling northbound. The Dodge was driven by Jeremy M. Wetherald, 50, of Lacey, Washington, with passenger Judith E. Holden, 73, of Hoquiam.

The collision caused the first vehicle to rest across both lanes of the highway, blocking northbound and southbound traffic. The second vehicle rolled and came to rest down an embankment.
Wetherald (Lacey) was pronounced deceased at the scene. Holden sustained injuries and was transported to Community Hospital for treatment. Mullikin was also injured and taken to Willapa Harbor Hospital. All parties were wearing seatbelts at the time of the crash.
Troopers noted that drugs or alcohol were a factor in the incident, though the investigation remains ongoing. Both vehicles were totaled and towed from the scene by Hoquiam Bullpen Towing.
